In this insightful and empowering episode of Chronic Boss Unfiltered, host Lilly Stairs sits down with Dr. Kara Wada — quadruple board-certified physician, autoimmune warrior, and founder of the Immune Confident Institute. As both doctor and patient, Kara bridges two worlds with empathy, expertise, and authenticity.
Diagnosed with Sjogren’s syndrome after years of symptoms and normal lab results, Kara experienced firsthand how challenging it can be to navigate chronic illness — even as a medical professional. What began as a personal health struggle has evolved into her mission to transform the patient experience and redefine what it means to thrive with autoimmunity.
Together, Lilly and Kara unpack the intersection of medicine, mindset, and self-advocacy. They discuss Kara’s Four Rs framework for healing, the “Perfect Storm” concept behind autoimmune disease, and how Kara built a values-driven medical practice rooted in compassion and connection. This episode is a masterclass in resilience, authenticity, and reclaiming your health on your own terms.
What You’ll Learn:
What happens when a quadruple board-certified physician becomes the patient? For Dr. Kara Wada, it sparked a personal and professional revolution.
In this episode of Chronic Boss Unfiltered, host Lily Stairs sits down with Kara to unpack the story behind her Sjogren’s diagnosis, her decision to leave a traditional health system, and the mission behind the Immune Confident Institute.
Kara’s journey took a dramatic turn after returning from maternity leave in 2019. As she put it, the shift was like peeling an onion—layered and full of tears. After years of symptoms like dry eyes, extreme fatigue, and body aches, a chance comment from her dental hygienist about her dry mouth triggered a realization: this might be something more.
Her intuition as a trained immunologist led her to request labs…and despite initial pushback from her primary care doctor, the results came back clearly abnormal. She was diagnosed with Sjogren’s, an autoimmune condition that affects moisture-producing glands but can also cause systemic issues.
“I have ultimate privilege in my position,” Kara explains, “and I shudder to think where I would be if my labs had been normal. I might still be undiagnosed.”
Kara’s firsthand experience with autoimmune disease has transformed the way she practices medicine. Beyond deepening her clinical knowledge, it shifted her mindset.
“I’ve always liked hearing stories, but now I need to listen,” she says.
“Most patients with chronic illness don’t read like a textbook.”
This insight led her to build a new kind of practice, one that prioritizes time, human connection, and individualized care. Through the Immune Confident Institute, Kara combines traditional medical training with integrative strategies to help patients rebuild their health.
One of Kara’s signature teachings is her “Perfect Storm” model. In it, she breaks down the layered biology of autoimmune disease: gut permeability, microbiome disruption, and overstimulation of the immune and nervous systems.
“We’re living in a modern world that chips away at our body’s barriers,” she explains. “And when those systems break down, it sets off a cascade of immune dysfunction.”
Kara also developed a patient-centered model she calls the 4Rs:
Through this framework, Kara empowers her patients to move beyond survival mode and into transformation.
Kara’s path hasn’t been without growing pains. Leaving a salaried role to open a private clinic has meant navigating hiring, scaling, and time management—often while overbooked and overstretched.
“I haven’t been doing a great job with my boundaries lately,” she admits, reflecting on the demands of running a practice while raising a family and managing her own health. “But I’m learning, I’m building, and I’m getting the right support in place.”
She also emphasizes the power of mentorship and community, citing the Chronic Boss Collective and other peer networks as key to her growth.
“To me, being a chronic boss is about showing up with the same tenacity that these conditions show up with,” Kara says. “It’s pushing back. Rewriting the rules. It’s knowing that even on the hardest days, I’ve survived worse—and I’ll rise again.”
