Eligibility Requirements
To be eligible for the Chronic Boss Scholarship, applicants must:
Media Consent and Publicity Release
If selected as a scholarship recipient, I grant Chronic Boss Collective the irrevocable right to use my name, likeness, submitted video, and any written content from this application (including excerpts from my story) for promotional, marketing, educational, and public relations purposes.
This may include, but is not limited to, publication in press releases, social media, websites, newsletters, and digital or print marketing materials. I waive the right to approve or inspect the final use and understand that I will not receive additional compensation. I release Chronic Boss Collective and Patient Authentic from any liability related to this content use.
Tax Implications Acknowledgment
I understand that if I receive the Chronic Boss Scholarship, the funds will be awarded as an unrestricted grant, which I may use at my discretion. I acknowledge that this may be considered taxable income under U.S. law, and I am solely responsible for reporting it on my tax return if required.

Eligibility Requirements

- Be currently living with a chronic illness
- Have already started a business, project, club, nonprofit, blog, or creative initiative
- Be enrolled in an accredited school, college, or university at the time of application
- Be a legal resident of the United States
- Be at least 18 years of age or older at the time of application
